Application Instructions
Doctor's referral or self referral required for one-on-one appointments. Contact for information or fill out the online referral form. Bring a list of current medications, blood sugar meter, log book of blood sugar readings, and health card to the appointment.
No referral required for the group-education classes or pre-diabetes program.
