Centre de Santé Communautaire Diabetes Education Program

Offers education and support to clients who have diabetes. 

Workshops and activities include: 

  • Understanding, preventing, and managing diabetes
  • Healthy cooking and eating classes
  • Benefits of physical activity
  • Nordic walking groups
  • Community screening clinics
  • Preventing complications and foot care
  • Stress management
  • Educational games
  • Cards and conversations
  • Ontario Telemedicine Network (OTN) remote education sessions

Diabetes clinics are also available for clients who are diabetic, prediabetic, or at risk of diabetes to meet with their doctor, a nurse, and a dietitian at once.

Eligibility

All francophone people age 18 years and older with diabetes, pre-diabetes, or at risk of diabetes in the Hamilton and Niagara regions are eligible to participate in the Diabetes Education Program, whether or not they are clients of the Centre de Santé Communautaire medical team. Referrals can be made by family physicians or nurse practitioners.
