Eligibility

To be eligible for this service, the person referred needs to be:

  • Living in Hamilton
  • 16 years of age or older
  • Diagnosed with a serious and persistent mental illness or concurrent disorder
  • Under the care of a physician (some services require a Hamilton physician)
  • Willing and able to participate in working on goals
