Application Instructions
Global Appraisal of Individual Needs (GAIN) and/or Admission and Discharge Criteria and Assessment Tool (ADAT) must be completed with potential client. These assessments can be completed by a counsellor in a variety of addiction service settings.
The minimum required documents for a completed referral are:
- GAIN or ADAT, including consents to release information by referral source
- Standardized Residential Services Referral Form
- Medical Evaluation Form
The assessment package should then be emailed, faxed, or hard copy mailed to Wayside House for review.
