A free community based pulmonary rehabilitation program designed to enhance quality of life for people living with Chronic Obstructive Pulmonary Disease (COPD) and help reduce hospital admission. Program participants will work with a team of healthcare providers, which includes a Certified Respiratory Educator, physiotherapist, kinesiologist, occupational therapist, social worker, and a dietitian. The program offers a bundle of services throughout a 10-week exercise and education program, followed by team and peer support for graduates of the program.
